Once you complete and submit your AmeriCorps Exit Form, some of the information you submit will be reviewed by your grantee organization. The following information will not be visible to your grantee organization:
Responses to the Member Exit Survey. All responses to the Member Exit Survey are anonymous. Survey responses will be pooled and used for reporting purposes, and combined survey results may also be shared with the public.
Responses to disability questions. Your disability status will never be provided to anyone in a manner where you can be identified. It will only be accessible by designated officials, and for the sole purpose of identifying which types of disability groups are serving in AmeriCorps. Your responses to disability questions will not be shared with your grantee organization or most staff in AmeriCorps or the Corporation for National and Community Service.
