As an AmeriCorps member, you will receive an e-mail thirty days prior to your expected completion date notifying you that the AmeriCorps Exit Form is available for you to complete. When you log into My AmeriCorps, go to the section at the top of your home page, entitled "Close of Service Forms." Complete this form once it becomes available to you and prior to your expected completion date.
