Will I need to complete the Exit Form if I am exiting early?

Yes. You will need to inform your grantee organization that you are exiting early in order for the grantee organization to provide you access to your Exit Form. If you have already informed your organization that you will be terminating your service early and still do not have access to the Exit Form, contact your grantee organization immediately.
