Manage Member Exit Survey
Welcome to the Manage Member Exit
Survey screen. From this main screen, you can perform a number of key
functions and navigate to other screens. You can also view summary information
about the survey’s questions.
Screen
Functions and Navigation
You can perform a number of functions
on this screen, including:
- Survey
Instructions –
Edit overall survey instructions (2000 character limit).
- Category
–
Change which categories (and all associated questions) are displayed.
- Switch
between categories by using the “Category” dropdown menu located
beneath the survey instructions.
- You
can also choose to “Show Active Questions Only” by clicking the
checkbox.
- Apply
these changes by clicking the “View” button.
- Pg
# – Assign
or change question page numbers by using the dropdown menu in “Pg
#” column.
- The
range of page numbers available in the dropdown menu is defined
in the Manage Categories
screen.
- If
there are any gaps between selected page numbers, no blank pages
will be displayed to the member.
- Note:
These page numbers defined in the Staff Portal may not necessarily
reflect the page on which questions display in the Member Portal.
For instance, this could be due to inactivated questions or because
one page applied to only one program.
- Seq
# – Assign
or change sequence numbers by typing the desired sequence number in
the small text box in the “Seq #” column (four digit limit).
- The
sequence numbers for each page begins at 1. For example, if the
first question on pg. 1 has a sequence number of 1, the first
question on pg. 2 can also have a sequence number of 1.
- Gaps
between sequence numbers are permitted.
- Active
–
Activate or inactivate questions by clicking the checkbox in the “Active”
column.
- Active
questions are visible to members.
- Inactive
questions are hidden from members.
- edit
–
Edit an existing question by clicking the “edit” link to navigate
to the Add/Edit
Member Exit Survey Question
screen and modify the question.
- Add
New Question –
Add a new question to the survey by clicking the “Add New Question”
button at the bottom of the screen. This
will navigate to an empty Add/Edit
Member Exit Survey Question
screen where you will be able to create a new question.
- Save
–
Confirm any changes to the main screen (such as edits to the overall
survey instructions, changes in a question’s page or sequence number,
or activating/inactivating questions) by clicking the “Save” button
at the bottom of the screen.
- Note:
Clicking “Save” on the Manage Member Exit Survey screen will trigger
the “Publish” button
to appear on the bottom of the screen.
- Cancel
–
Undo any changes to the main screen by clicking the “Cancel” button
at the bottom of the screen.
- Preview
–
Generate a preview of the survey by clicking the “Preview” button
in the “Preview Survey” box at the bottom of the screen.
- The
survey preview is displayed in one continuous HTML view, but will
have lines and page numbers showing where the page breaks occur
and on what page each question appears on the Member Portal. It
includes all active questions, including dependent questions.
- Make
sure to first select the desired program from the dropdown menu.
- Publish
–
Publish a new survey.
- Adding
a new question, editing a question, or inactivating a question
will create a new version of the survey. Once changes are confirmed
by clicking the “Save”
button on the Manage Member Exit Survey screen, a “Publish”
button will appear at the bottom of the screen.
- Do
not click this button until you are ready to publish a new version
of the survey. Publishing a new version of the survey will make
it live to members.
- If
a member is in the process of completing their survey when a new
version is published, he or she will continue to see the previous
version when they return to complete their survey.

Navigate to the Manage Categories
screen by clicking the “Manage Categories” link below the “Category”
drop down field.

Navigate to the Add/Edit Member Exit
Survey Question screen
by clicking the “edit” link on the far right side of the screen. You
can also go to this screen by clicking on the question text under
the “Questions” heading.
- Add
New Question –
Navigate to the Add/Edit Member Exit
Survey Question screen
by clicking the “Add New Question” button at the bottom of the screen.
- View
Historical Versions –
Navigate to the View Historical Versions
screen by clicking the “View Historical Versions” link in the “Preview
Survey” box at the bottom of the screen.
List

- QUEST
ID – This
four digit number is automatically generated when a new question is
saved.
- Questions
–
The beginning of the primary question text is displayed here. Clicking
on this question text will open the Add/Edit Member Exit
Survey Question screen.
If an asterisk (*) appears
before the question text, this means that this question was created
or edited after the publication of the most recent survey.
- QUEST
Type
- PO
– “Primary Only” – A question with no dependent questions or sub-questions.
- PD
– “Primary Dependent” – A question with at least one response
that triggers a dependent question.
- PS
– “Primary Sub-Question” – A question with sub-questions.
- Pg
# - The page
number assigned to this question. See discussion above.
- Seq
# - The sequence
number assigned to this question on a given page. See discussion above.
- Active
–
This checkbox indicates whether or not this question is active and
therefore displayed to members.
- Response
Type – The
following response types are available options on the Add/Edit
Member Exit Survey Question
screen.

Survey Questions is designed to provide an overview of the questions.
In order to make most types of changes (including deleting a question
that has not been published), the user must navigate to the Add/Edit Member Exit
Survey Question
screen.
