This section outlines instructions for adding new questions and also editing existing questions (both published and unpublished). The heading on this screen reflects the status of the given question:
Add New Member Exit Survey Question – A new question for which information is being entered for the first time.
Edit New Member Exit Survey Question – An existing question which has not been published.
Edit Member Exit Survey Question – An existing question which has been published.
Differences in screen functionality based on the question’s status are noted below. Most notably, questions that have been published (and all associated dependent or sub-questions and responses) cannot be deleted, though they can be inactivated.
Basic Question Information
The following is a list of question components applicable to all question types:
Programs – Select the corporate program(s) to which this question applies. You may select one program or multiple programs. You may select all programs by clicking the “ALL” checkbox.
Category – Select the category in which the question should appear. This dropdown menu is populated by the categories created on the Manage Categories screen.
Optional Instructions – This field can be left blank (2000 character limit).
QUEST ID – This four digit ID is generated automatically once the question is saved.
Question Name – Type a short label for this question (50 character limit). This cannot be edited after a question has been published.
Question – Enter question text here (2000 character limit). Once a question has been published, the question text cannot be changed.
QUEST Type – Choose between the three options below. Once a question has been saved, the question type cannot be changed.
Primary – A question with no dependent questions or sub-questions. This “QUEST TYPE” will display as PO on the Manage Member Exit Survey screen.
Dependent –A question with at least one response that triggers a dependent question. This “QUEST TYPE” will display as PD on the Manage Member Exit Survey screen.
Sub-Question – A question with sub-questions. This “QUEST TYPE” will display as PS on the Manage Member Exit Survey screen.
Active – This checkbox indicates whether or not this question is active and therefore displayed to members. Questions are active by default.
Save – Click this button to save entered information and refresh the screen.
Note: In order to confirm changes and trigger the display of the “Publish” button, the “Save” button must also be clicked on the Manage Member Exit Survey screen.
Cancel – Click this button to refresh the screen without saving any changes.
Back – Click this button to navigate back to the Manage Member Exit Survey screen. Make sure to save all desired changes before doing so.
Delete – Click this button to delete the entire question. This option is only available for questions that have not been published.
QUEST Type: Primary
The following fields and functions apply to primary questions that do not have dependent questions or sub-questions.
Response Required – This checkbox indicates whether or not this question is required. Questions are required by default. A required question will be displayed to members with an asterisk (*).
Response Type - The following options below for this question type (Primary). Once a question has been published, response type cannot be changed.

Numeric Textbox (13 digit limit, decimals allowed)
Textbox Large (1500 character limit)

Seq # - Indicate the sequence number for this response (four digit limit).
Active – This checkbox indicates whether or not this response is active and therefore displayed to members. Responses are active by default.
delete – Click this link to delete a response. Once a response has been published, it cannot be deleted, though it can be inactivated.
QUEST Type: Dependent
Response Required – This checkbox indicates whether or not this question is required. Questions are required by default. A required question will be displayed to members with an asterisk (*).

Dependent – This checkbox indicates whether or not this response has a dependent question. Checking this box will make the “Add QUEST” link display to the right of the checkbox.
For each question, up to two responses can have dependent questions. In turn, these two responses can have unlimited dependent questions.
If a question is published with two responses that have dependent questions, no additional responses can have dependent questions added at a later date. Existing responses with dependent questions can be inactivated, but new responses with dependent questions cannot be designated in their place.

delete – Click this link to delete a response. Once a response has been published, it cannot be deleted, though it can be inactivated.
