Enter your current status as either U.S. Citizen, U.S. National, or Lawful Permanent Resident Alien. If you are lawful permanent resident alien, and you received your card after January 1987, enter your registration number and expiration date.
Next, be sure to enter your City of Birth, State of Birth (if applicable), and Country of Birth. Proceed to enter your gender: male or female.
Then you must fill out the Earliest Availability Date, the earliest date that you would be available to begin service on an AmeriCorps project. Make sure to enter in the correct format (mm/dd/yyyy), for example 04/14/1965.
Finally, you must state whether or not you plan on moving within the next six months. If you are planning on moving, please specify the expected date of the move.
