Enter your name exactly as it appears on your government-issued identification (e.g. driver’s license, passport, etc.). If a middle initial/name appears on your government-issued identification, enter it in the Middle Name field. Enter your Social Security Number (SSN), a nine digit number issued to citizens, nationals, or lawful permanent resident aliens. You must enter your date of birth (mm/dd/yyyy), for example 05/16/1982. You are also required to enter a valid e-mail address to register because the system will send you a link via e-mail to establish your username and password for the system. If you do not currently have an e-mail address, search the Internet for a service that provides free e-mail addresses.
