What type of documents should I attach?

You need to attach any documentation that can support your request for extension. For example, military records, death certificates, newspaper obituary notice, or a letter from your doctor.  If you have any questions about what to attach, please contact the Trust at 1-800-942-2677.

 

The acceptable documentation formats to upload are: .txt (Text file); .doc (Word document); .docx (newer version of Word); .xls (Excel); .xlsx (newer version of Excel); .pdf (Portable Document Format); .ppt (PowerPoint); .pptx (newer version of PowerPoint); and .jpeg (image).
