How do I attach my substantiating documentation?

In order to submit your Award Extension request through the online system you need to electronically attach all of your substantiating documentation. If you cannot attach the documents electronically, print out the Award Extension page, sign it, and mail the signed form and all of your substantiating documentation to CNCS, AFMS: Trust Office, 250 E Street SW, Suite 300, Washington, DC 20525. Or you may fax the request and all supporting documentation to the Trust at 202-606-3484. It is recommended that you keep copies of all documentation for your records.
