What happens if the "Action approved by CNCS” in Part C of my Future Plans/Exit Form does not match the action I requested in Part A?

The State Office may or may not approve an extension or re-enrollment request depending on various factors such as AmeriCorps VISTA resources and project needs. For example, if you requested to extend your service or re-enroll for another year, but "Regular Exit" is displayed as the "Action approved by CNCS" in Part C of your completed Future Plans Form, you will be exited from VISTA service on your scheduled completion date. If you have further questions, please contact your State Office as soon as possible.

If you requested an early termination or regular exit in Part A, that same action should be marked as "Action approved by CNCS" in Part C. If this is not the case, you should contact your State Office immediately.
