What are the different statuses of the Future Plans/Exit Form and what do they mean?

Your Future Plans/Exit Form will go through a number of different statuses before you end your service. If you select to receive e-mail notifications as a member, you will be notified by e-mail each time the status of your Future Plans/Exit Form changes so that you can track the completion of the process.

Every form will go through 5 statuses in all:

Incomplete: When the Close-of-Service Forms first become available to you, the Future Plans/Exit Form will be listed as "Incomplete" until you complete and submit the forms.

Pending Sponsor Review: Once you successfully submit the forms, the status will change to "Pending Sponsor Review." At this time, your Future Plans Form only will be sent to your supervisor for review. The form will remain in this status until your supervisor conducts your exit interview, and completes and certifies the sponsor portion of the Future Plans Form.

Pending Member Review: Once your supervisor completes and certifies your Future Plans Form, the form’s status will change to "Pending Member Certification". At this time, you will log into the My AmeriCorps portal and access the Future Plans/Exit Form again. This time, you will be able to view your supervisor’s evaluation and recommendation. You will also have the chance to comment on your sponsor’s feedback. You must certify that you have reviewed the form before it can proceed to the next status.

Pending State Office Review: Once your supervisor and you both completed the review and certification of the Future Plans Form, your State Office will perform a final review. The State Office staff will read over the form in its entirety, including both the member and sponsor portions and select to either approve or deny your requested action.

Complete: Once the State Office reviews and completes your Future Plans Form, the form’s status will change to its final status, "Complete". At this time, you will receive an e-mail notification instructing you to log into My AmeriCorps to review the action approved by the State Office on your Future Plans Form.
