The Future Plans Form is made available to all full-time VISTAs once they reach the beginning of their tenth month of service. This form may also be made accessible to a member earlier than this date on a case-by-case basis, as necessary (usually due to a planned early termination). Please note that Summer Associates always have access to this form during their term.
Once the member completes and submits the member portion (Part A) of the form, the form will appear in your Future Plans Form workbasket with a status of “Pending Sponsor Review”. You may then review the form in preparation for the exit interview. You should not complete and submit the sponsor portion (Part B) of the form until after you conduct the exit interview with the VISTA (except in rare instances when exit interviews are not possible because, for example, the individual’s whereabouts are unknown or s/he is otherwise unavailable).
Once you complete Part B and certify the form, it will be removed from your Future Plans Form workbasket. The form will then be accessible to the member for review and certification. Once the member certifies the form, it will appear in the State Office’s Future Plans Form workbasket for final review. See below for explanations of each Future Plans Form status:
Pending Sponsor Review: Once the member submits Part A of the form, it will appear in your workbasket with this status. The form will remain in this status until you complete and certify the sponsor portion (Part B) of the Future Plans Form.
Pending Member Review: Once you complete and certify a VISTA’s Future Plans Form, the form’s status will change to “Pending Member Certification”. At this time, the member will be able to view your evaluation and recommendation, as well as comment on your feedback if he or she desires. Until the member certifies the form, it will remain in this status.
Pending State Office Review: Once you and the VISTA both certify the Future Plans Form, it will be forwarded to the State Office for final review. The State Office staff will read over the form in its entirety, including both the member and sponsor portions. The State Office will then select to either approve or deny the VISTA’s requested action based on your evaluation and recommendation.
Complete: Once the State Office reviews and indicates the approved action for the member’s end of service on the Future Plans Form, the form’s status will change to its final status, “Complete”
