Your Future Plans Forms workbasket will display all forms submitted by your members for your review. All VISTAs will need to complete the Future Plans Form and Exit Form prior to finishing their term of service. This form gives members an opportunity to formally request to end their term of service as scheduled, re-enroll for an additional year, extend service on current project, or terminate service early. It also gives you the chance to complete an evaluation and recommendation for the VISTAs serving under you. Ultimately, this form will be used by the State Office Staff to determine whether a VISTA should be exited, re-enrolled, or extended at the end of his or her term of service.
