What is the V-81 Transportation Form?

The AmeriCorps VISTA “Use of Vehicles and Public Transportation Form” (Form V-81) documents that a vehicle is necessary for you to perform your VISTA assignment. If you plan on using any of the service-related transportation listed below, you must complete Form V-81. It will automatically be submitted to your supervisor for initial approval and then to your State Office for final approval. This form addresses service-related transportation at site, not transportation for personal use. (Daily commutes to the service site are not considered a service-related need.)

1. Vehicle owned or leased by your sponsor organization.

2. Vehicle owned or leased by you.

3. Vehicle owned or leased by another person and approved for your use by the Corporation State Director.

4. Public Transportation.
