Proceed to the "Insurance Information" section, checking the box provide, certifying that your vehicle is licensed and insured as required by State and local laws.

Move onto the "Purpose of Travel" section, where you shall provide an explanation of all proposed travel that you plan on having to do with the project-owned vehicle while serving on your AmeriCorps project.

Once you have completed the information, review the information enter to make sure of its accuracy, and then submit the form by clicking the button at the bottom of the page. You may also print this form for your own records, by clicking on the "Print this Form" link.
