Proceed to fill out the "Personal Vehicle Information" section including: your vehicle make and style, the year of your vehicle, the state in which it is registered, your license number, the name of your insurance company, the person who is responsible for paying for the operating costs of the vehicle (whether you or someone else), the amount and type of coverage you have with your insurance, the average number of miles you travel monthly, the primary driver of your vehicle, and the names of any other driver that use your vehicle (if applicable).
Under the "Insurance Information" section, indicate by checking the box, certifying that your vehicle is licensed and insured as required by State and local laws.
Under the "Purpose of Travel" section, provide an explanation of all proposed travel that you plan on having to do with your personal vehicle while serving on a VISTA project.
Once you have completed the information, review the information entered to make sure of its accuracy, and then submit the form by clicking the button at the bottom of the page. You may also print this form for your own records, by clicking on the "Print this Form" link.
