Purpose
The AmeriCorps Member Exit Survey is an important way that you can make your voice heard as an AmeriCorps member. Your responses help us learn about how programs are doing and what can be done to improve them. Thank you for taking the time to complete the survey.
General Instructions
An asterisk (*) denotes a required question. You must respond to this question in order to move on to another page.
IMPORTANT: As you complete the survey, you are welcome to exit and return to the survey as needed. However, please remember to save your responses before exiting the survey. Changes will not be saved if you navigate away from the survey without first clicking the “Back,” “Next,” or “Save” buttons.
Back – This button saves your responses on the current page and returns you to the previous page.
Save – This button saves your responses on the current page.
Next – This button saves your responses on the current page and moves you on to the next page
Submit
– This button, visible
on the last page, submits your responses to the Member Exit Survey.
Responses cannot be changed once the survey has been submitted.
