NNCC Expansion of NFP Community Resource Network
 
 
 
The National Nurse-Led Care Consortium and NFP have a vision to scale up strategically over the next six years to make a community-level impact on the school readiness of Philadelphia kindergarteners. NNCC will support a planning process to grow towards making a community-level impact on the number of children who enter school ready for kindergarten. As we plan to scale up, we intend to engage in a thoughtful process that includes our partners. Our experience makes clear that rapid growth in home visiting services does not occur in isolation. The programs require intensive partnerships—to bolster the workforce pipeline, recruit families, and to link families with other community resources—and strong infrastructure to sustain and support staff. NNCC will also strengthen staff orientation to the organization for integrating new remote employees. NNCC is committed to the families we serve. Together we can achieve outcomes that are meaningful to our community and society—preparing more children to have lifelong success. Thus, we have a dedicated infrastructure that prioritizes identifying unmet needs experienced by families and pursuing opportunities to approach healthcare delivery in innovative ways.

Member Duties : 1. Develop a mechanism for home visitors to provide feedback on support provided by home visiting navigators to guide program improvement; 2. Develop and routinize system of tracking community resources and early childhood outreach contacts and updating new information; 3. Research additional community resources for families are identified and updated; 4. Assist in developing protocols for supporting all NNCC home visitors and clients to connect to community resources 5. Support development of NNCC orientation for new staff and nurses 6. Research funding opportunities and support with grant applications as needed
