Oregon Rural Accountable Health Communities VISTA
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. The Oregon Rural Practice-based Research Network (ORPRN) was established in 2002 at Oregon Health & Science University in Portland. ORPRN is dedicated to improving the health of rural Oregonians through community-engaged dialogue, coaching, education and research. ORPRN facilitates a broad variety of clinical health care research, quality improvement projects, and practice transformation efforts in settings serving rural and underserved populations in Oregon. The Accountable Health Communities project’s goal is to identify the social needs patients on Medicare and Medicaid and assist them with accessing services to resolve their needs. By improving access to health care and social services, these two programs position participants to transition out of poverty. Member Duties : The Accountable Health Communities project seeks to develop integrated, voluntary systems across the health care, public health and social service sectors to identify the social needs of medically underserved populations in rural areas and to assist these populations with accessing services to resolve their needs. The VISTA member will increase understanding of and participation in the Accountable Health Communities model by social service agencies. VISTA member activities include: o Reviewing data on which Medicaid and Medicare patients are accessing most for social services o Identifying shortcomings in the services available to Medicaid and Medicare patients, including unmet needs in housing, transportation, utilities, safety, and food security o Developing and implement an outreach plan to encourage patient participation on steering committees o Reporting findings to social service agencies and project leadership, including the Statewide Advisory Board & steering committees
