E3.20 Community Health Specialist VISTA St. Louis County
 
 
 
Facilitate true collaboration among health care systems, public health, human services and the nonprofit sector in our community, a joint community health needs assessment process was developed and conducted in northeastern Minnesota. Organizations including St. Louis County Public Health, nonprofit hospitals Essential Health and St. Luke's, Generations Health Care Initiatives, Zeitgeist Center for Arts and Community, and Lake Superior Community Health Center, have aligned their resources, skills, expertise and interests to collaborate towards a healthier community. The project is designed to identify and reduce health disparities by addressing the social determinants of health.

Member Duties : In order to address the community health priorities (mental health, substance use, and social determinants of health) in St. Louis County, MN, the Ecolibrium3 VISTA Corps project will build capacity of St. Louis County's Public Health and Human Services. The member will work with existing partnerships with local agencies to enhance coordination and support of community health promotion efforts. Specifically the member will: 1) support the development and growth of Bridging Health Duluth, a multisector community health assessment and planning Collaborative. 2) Build or improve Bridging Health Duluth processes around data, evaluation, and performance management. 3) Expand impact of Bridging Health Duluth through Community Engagement and Outreach planning and implementation. 4) Build personal skill and knowledge to serve as a countywide resource through successful completion and application of training.
