NNCC Infectious Disease Surveillance Expansion
 
 
 
The VISTA will support NNCC's efforts to reduce poverty by addressing the root causes of health disparities. Activities will include 1)Developing a network of organizations to schedule community testing and vaccination events that will target priority communities, while patient navigation will occur to support the clinic sites, the testing events, and a novel data-to-care approach of using PDPH surveillance data to identify out of care individuals and perinatal HBV mothers for linkage services. 2) Reviewing the needs assessment from Year 1 of the project to identify gaps and barriers that must be overcome to eliminate HBV in Philadelphia. 3) Partnering with the Hepatitis B Foundation to create educational materials and establish protocols for routine screening and linking patients to care at health centers in Philadelphia. The main goals for the VISTA will be supporting the project to reduce health disparities for people living with HBV at the citywide, clinic, and individual level, and develop sustainable strategies, including strengthened data systems for addressing HBV patient engagement in Philadelphia.

Member Duties : The member tasks and activities will include: 1. Summarize HBV needs assessment in Philadelphia for key project stakeholders. 2. Assist in development of quality improvement plans for data collection on HBV in PHMC. 3. Create HBV educational presentations for community members in risk groups. 4. Assist in coordinating event opportunities for HBV education in the community. 5. Assist in creation of HBV educational materials and events for PHMC health center providers. 6. Develop database of collected best practices for distribution to Philadelphia health centers. 7. Compile summary report of patient and provider survey responses for PHMC health centers.
