Oregon Primary Care Equity and Quality Improvement VISTA
 
 
 
This position joins the Oregon Health Authority’s statewide VISTA team. In addition to activities at your individual site, each member receives additional orientation, training throughout the year, team retreats and meetings, and the support of the team leaders and project director based at the state public health agency. OPCA is a nonprofit membership association of Oregon’s community health centers, also known as federally qualified health centers (FQHCs), other safety net clinics, and those who support them. Our mission is to lead the transformation of primary care to achieve health equity for all. OPCA provides technical assistance, training and support to health centers. OPCA partners with health centers to advance the goals of health system transformation: better health, better care, lower costs and health equity. Challenges such as poverty and homelessness, cultural and language barriers, rural or social isolation, and chronic mental illness often have a significant impact on our patients’ health. In addition to helping health centers provide high quality integrated medical, dental and behavioral health care, much of our work involves helping providers better understand the circumstances of their patients’ lives and address the social determinants of health as a major driver of health equity. Together with health centers across the state, we work to understand and help address the social determinants of health in the clinic, and through community partnerships. Member Duties : This VISTA project will increase access to primary care services at Oregon’s 32 community health centers by expanding the collection and sharing of Social Determinants of Health assessment data and strategies to connect with social service agencies. This program will expand the ability of health centers throughout the state to measure, understand and address the social and environmental factors known as the Social Determinants of Health (SDoH) that contribute to poor health outcomes among the most vulnerable Oregonians. The VISTA will research national and statewide best practices and share the results among Oregon’s community health centers and allow us to better evaluate the impact of that work to inform and improve those strategies in the future.
