Integrating Health and Social Care – C4P at Rush
 
 
 
Rush University Medical Center is a large integrated academic medical center that serves patients and community members on the west side of Chicago. Rush has several care management and community engagement initiatives seek to provide whole-person health care to Rush patients, families, and community members, including a social determinant of health initiative that screens individuals for unmet social and economic needs, and provides follow-up resources and care management. Rush is proud to partner with The Comprehensive Care, Community and Culture Program (C4P) at the University of Chicago to enhance our program by hosting three C4P AmeriCorps members at Rush. The Rush members will participate in Rush initiatives that are aligned with but have distinct protocol from the University of Chicago’s C4P program.

Member Duties : C4P @ Rush AmeriCorps members will be responsible for extending the reach of Rush’s care teams to improve patients’ care experience and health outcomes by identifying and addressing unmet social needs for participants. The Rush AmeriCorps members will assist with several tasks related to identifying and supporting social determinants of health needs: • Conduct social determinant of health screenings as part of proactive outreach and other initiatives integrated within Rush health care teams • Utilize electronic health record and a social service referral platform to track screening responses and initiate navigation of resources • Conduct follow-up screens and provide light navigation for patients previously screened from, the emergency department, inpatient, and the community • Provide additional support for community initiatives and health promotion workshops.
