2019-2020 Family Care Coordinator VISTA
 
 
 
The Soutside Community Center focuses on community development by transforming community one relationship at a time. We employ Asset Based Community Development Principles to address economic development project in Peoria Southside Zip code, the second most economically depressed community in Illinois. SCC and this VISTA will create a collaborative system of care we call the Pathway of Hope to support families toward self-sufficiency, living wage employment and first-time home ownership.The VISTA will design and implement a strategic program to collaboratively support children and their families while simultaneously leveraging relationships to develop local leadership from parents to help them plan their own future goals to become self-sufficient members of our community. This position is a part of the Housing Action VISTA Network, which is an AmeriCorps VISTA program managed by Housing Action Illinois since 2010. As such, this position will receive expert training, support, guidance, and networking opportunities, in addition to the traditional opportunities for AmeriCorps VISTA positions. They will need to be proficient in skills for collection, recording and analysis of data.

Member Duties : 1. Develop Collaborative System of Care bringing Faith-Based Organizations, Southside Residents and Service Providers to the discussion table. 2. Build relationship with Faith Community Leaders, Local Residents, and Social Work Providers to meet monthly. 3. Establish baseline for participants; youth and adult stress, trauma and social emotional health screening tools as part of the Intake process. 4. Develop timeline and process for benchmarking data and follow-up on screening and referral processes in the Pathway of Hope. 5. Identify a Cohort of 4 families to pilot the Pathway of Hope process. 6. Set goals with families through the Connect, Grow, and Serve Model. 7. Participate in developing and documenting a formal sustainability plan as part of the Evaluation Plan for potential funders. 8. Will be looking for potential grants to propose our work and research data to bring dollars to Southside Community.
