HealthReach Community Health Centers www.HealthReach.org, has been providing primary care to rural residents of all ages since 1975. Our mission is “to provide high quality affordable, patient-centered healthcare in the medically underserved communities of Central and Western Maine”. The AmeriCorps VISTA member will improve services while continuing the enhancement of geriatric self-sufficiency, ability to remain at home, and overall wellness. The additional focus will be designing outreach strategies to link low income families to programs that help reduce the cost of healthcare and medications and connect them to resources for nutritional food and social services.

Member Duties : The AmeriCorps VISTA member will expand and improve supportive services and partnerships, increase educational outreach, and develop resource guides for the geriatric population. They will recruit, orient, and support volunteers to assist with food distributions, and organize cooking classes and education curricula. The AmeriCorps VISTA member will also increase outreach efforts to vulnerable citizens and increase the capacity to enroll the public in health insurance plans.
