HealthReach Community Health Centers, a private non-profit organization, has been providing primary care (checkups for the entire family, care of acute and chronic illnesses, integrated behavioral health, laboratory services, minor surgery, as well as referrals to specialty care and community resources) to rural residents of all ages since 1975. 46% of the 27,998 patients who utilize health center services are below 200% of US poverty guidelines. For primary health insurance, 25% of patients have Medicare, 19% Medicaid and 24% have no insurance. Our mission is “to provide high quality affordable, patient-centered healthcare in the medically underserved communities of Central and Western Maine” through 11 Federally Qualified Health Centers serving 80 towns in parts of 9 counties. The AmeriCorps VISTA member’s foci will include assisting with the launch of an initiative to improve services while continuing the enhancement of geriatric self-management of chronic conditions, improvement in overall wellness, and increased low-income seniors’ ability to remain at home. The additional focus will be designing outreach strategies to link low income families to programs that help reduce the cost of healthcare and medications and connect them to resources for nutritional food and social services.

Member Duties : Geriatric population assignments: • Research/expand supportive services • Improve medical-legal partnerships • Develop advanced care planning processes • Increase educational outreach • Set up chronic disease management classes • Expand capacity to assist low income population with healthcare insurance enrollment (Marketplace, Medicaid, Medicare, Veteran services) • Develop local physical activity resource guides • Research/develop health tips/educational pieces for internal and external audiences to promote wellness Low-income family initiatives: • Recruit, orient and acknowledge volunteers to assist with food distributions • Develop health education curricula for local schools • Organize cooking classes • Become a Centers for Medicare and Medicaid Services Certified Application Counselor and increase outreach efforts to vulnerable citizens and capacity to enroll the public in health insurance plans Design education displays for health center waiting rooms
