Delaware Valley Community Health VISTA
 
 
 
The primary focus of the Delaware Valley Community Health service project will be access to Health Care. The VISTA member will split their time between Maria de los Santos Health Center (MdlS) and Fairmount Primary Care Center (FPCC). The VISTA member will be primarily responsible for creating and maintaining community relationships. The VISTA member will attend and represent DVCH at local and state level civic meetings, community events, and local CDC meetings. The VISTA member will work with neighborhood groups to provide space for meetings and organize town hall meetings to share DVCH information. The VISTA member will create a resource database to store and share this information with DVCH staff and members of senior management. The VISTA member will work closely with active patients in determining direction and focus of projects to ensure patient engagement.

Member Duties : 1. Develop and maintain a community partnerships 2. Create a needs assessment tool to identify the wants and needs of the community to be used for health center engagement projects 3. Attend neighborhood and local government meetings 4. Establish the presence of Delaware Valley Community Health in the community 5. Attend health-focused events throughout the city 6. Engage current and potential patients and community partners 7. Develop long-lasting relationships that benefit our health centers and patients.
