PHMC Health Network VISTA
 
 
 
The PHMC Health Network VISTA will develop a portfolio of social service resources at the PHMC health centers to effectively connect patients to needed services moving forward. The VISTA will assist the health center’s registered nurse and social worker in eliminating barriers to care. The VISTA will identify gaps in care coverage and work within our interdisciplinary team to develop approaches to closing those gaps. The VISTA will utilize our electronic medical records to gather data to assist in developing education and support models for patient benefit to be administered by phone or in the office and evaluate these models for success. Additionally the VISTA would work closely with the PHMC Social Services Manager and Health Care Administrator to coordinate approximately twenty special, one-time events at the health centers including seasonal events, patient appreciation efforts, and patient education events. The VISTA would be responsible for categorizing, documenting, and scaling events at each health center so that they can be easily implemented at all sites.

Member Duties : 1. Develop, implement, and coordinate social service special events for the Mary Howard Health Center and the five other PHMC Federally Qualified Health Centers. 2. Coordinate programming of 20+ special events for patients and staff with support of the Social Services Manager and Health Care Administrator. 3. The VISTA would be responsible for categorizing, documenting, and scaling events at each health center so that they can be easily implemented at all sites 4. Identify, categorize, document, and scale special events at Health Network sites. 5. Develop and manage social service resources to effectively connect patients to needed services 6. Builds and maintains strong relationships with PHMC program partners, project staff, and other relevant stakeholders to ensure effective communication and integration of services. 7. Collaborate with Program Social Services Manager and health center staff to develop new program initiatives and monitor existing efforts
