PHMC Health Network Hep C Virus Coordinator
 
 
 
This VISTA project aims to meet health needs by increasing access to healthcare for economically disadvantaged individuals. As these individuals most often have Medicaid or Medicare, it's more difficult to obtain coverage for medically necessary medications, diagnostics procedures, and services. This lack of access to care contributes to health disparities and puts low-income individuals at higher risk of complications from chronic disease progression. This Hepatitis C Virus Coordinator position will assist with completing tasks related to obtaining approval for the treatment of Hepatitis C. With this program we have seen an increase in the number of patients who have been treated for hepatitis C virus, and they have been able to receive their treatment with their primary care provider instead of being referred to a specialist. The VISTA member will be able to support the continued development of systems that streamline the process from when a provider orders a medication/procedure/service, through getting it approved, and then scheduling/following-up as necessary to ensure the patient receives the service. Additionally, they will assist with developing a documentation process that ensures that patients are informed throughout the process and feel empowered in their healthcare.

Member Duties : 1. Establish relationships with offices and insurance companies, and creating internal processes to identify patients who need assistance. 2. Establish a system for requesting prior authorizations for non-formulary medications 3. Create a training system to appeal denials to insurance companies for necessary medications 4. Build and maintain relationships with PHMC program partners, project staff, and other relevant stakeholders to ensure effective communication and integration of services. 5. Build tracking system for HepC patients that are positive, starting treatment, are currently on treatment and have completed treated. 6. Support providers in process of authorizing HCV medications for patients. 7. Expand HepC treatment model to five other PHMC health centers by supporting the facilitation of provider trainings 8. Replicate HepC referral model to other chronic diseases treated at the PHMC health centers.
