The VISTA project aims to meet health needs by increasing access to healthcare for economically disadvantaged individuals. As these individuals most often have Medicaid or Medicare, it's more difficult to obtain coverage for medically necessary medications, diagnostics procedures, and services. This lack of access to care contributes to health disparities and puts low-income individuals at higher risk of complications from chronic disease progression. The VISTA will support data collection for funding and clinical engagements used by the Center of Excellence. The COE VISTA will work on collecting and organizing data from the electronic health records. In addition, the VISTA will support data collection and tracking measures that inform the clinicians’ approaches and supports the infrastructure of the MAT programming. The VISTA will help build the sustainability of expanding the MAT program within the PHMC Health Network.

Member Duties : 1. Assist in the distribution of the weekly MAT audit email for the PHMC Health Network 2. Develop tracking system for nursing staff to review the weekly active and inactive patients 3. Update referral list protocol to include all referral sources and send out to COE team weekly 4. Develop patient and provider surveys to evaluate progress of MAT expansion 5. Maintain and update survey completion results per month for team meetings 6. Maintain data collection of positive results to COE supervisor 7. Assist in grant writing for continued support of the MAT expansion program
