The Chronic Disease Management Prevention Capacity Building Project will partner with the PHMC’s Health Network (PHMC HN) and other partners, such as Einstein Health Network and Resources for Human Development Family Health Services to address the problem of uncontrolled diabetes and other chronic diseases among Philadelphia’s high risk populations.
