ShareCorps - CareNet - Communications VISTA

Toledo/Lucas County CareNet provides low-income Lucas County residents with access to coordinated healthcare services. Founded as a charity care network in 2002, CareNet now also employs community health workers (CHWs) to address the social determinants of health by connecting low-income residents to medical and social services, including food, transportation, housing and other needs. These CHWs also are certified application counselors who help Lucas County residents apply for Health Insurance Marketplace and Medicaid coverage.
