Economic Opportunities in Northern NM_Aging Programs
 
 
 
This project will identify gaps in services and develop an action plan to transform the delivery of health, nutrition and transportation services to low income seniors throughout rural communities in New Mexico. Over the course of the project the assigned VISTA will achieve four major objectives that, done well, will result in an opportunity to transform the way seniors receive critical services in rural communities throughout New Mexico. 1. The assigned VISTA will research existing data related to food insecurity, access to health care, transportation, demographic data- including income data, population data, incidence of chronic illness and other relevant data to determine critical needs of seniors in New Mexico. 2. The assigned VISTA will create and disseminate to seniors, and service providers a comprehensive needs assessment survey tool, conduct focus groups with service providers, seniors and other and solicit input from institutional leaders through a survey tool to identify gaps in services. 3. Work with local providers to build capacity in local needs assessment, and evaluation of costs associated with addressing gaps in services and unmet need. 4. Develop an action plan and associated collateral materials for increased funding for senior services from all potential funding sources.

Member Duties : The VISTA member will 1. Gather data related to food insecurity, access to health care, transportation, and other social determinants of health; 2. Gather demographic data related to income and aging in New Mexico 3. Develop and disseminate a comprehensive needs assessment survey tool for seniors, service providers, local governments and institutional leaders. 4. Conduct focus groups to solicit information regarding gaps in services, emerging needs or expansion of existing services. 5. Assist with modeling costs associated with addressing gaps in services. 6. Analyze data into a comprehensive report with action plan recommendations. 7. Develop collateral material associated with the report/action plan 8. Convene stakeholders to review and respond to the report and action plan. 9. Arrange for media coverage to highlight the findings in the report
