19-20 Renown Population Health Program Specialist
 
 
 
Nevada is one of the fastest aging states in the United States and the "Enhancing Well-Being and Health Outcomes Among Older Adults" VISTA project aims to build capacity for programs and services that support older adults. The Sanford Center for Aging, based out of the University of Nevada, Reno's School of Medicine is the sponsor for the project and is working with five community sub-sites to foster collaboration and expand the scope of supports and services aimed at enhancing quality of life for our aging community. Renown Health is the only locally owned, not-for-profit health system in Northern Nevada and is dedicated to making a genuine difference in the health and well-being of older adults. The Population Health Department at Renown focuses on key health determinants (i.e. socio-economic, lifestyle, and clinical determinants of health) to create community health interventions. The VISTA member will build capacity for these community health interventions.

Member Duties : The Population Health Program Specialist VISTA will be based out of the Population Health Department at Renown and supervised by the Director Community Care Management. The VISTA member will work to build capacity for community health interventions that focus on key health determinants (i.e. Socio-economic, food security, lifestyle, and clinical determinants of health). This will include collaborating with key community stakeholders, identifying best practices for community health worker and volunteer interventions, and conducting an audit on current community health protocols and procedures. The VISTA will receive direct support from the Director of Health and Wellness as well as the rest of the Population Health team. The VISTA member will design an optimal system for embedding Community Health Workers and Community Health Volunteers into a patient-centered system of care.
