Nevada is one of the fastest aging states in the United States and the "Enhancing Well-Being and Health Outcomes Among Older Adults" VISTA project aims to build capacity for programs and services that support older adults. The Sanford Center for Aging, based out of the University of Nevada, Reno's School of Medicine is the sponsor for the project and is working with five community sub-sites to foster collaboration and expand the scope of supports and services aimed at enhancing quality of life for our aging community. In Washoe County, Nevada adults aged 65 and older comprise 13% of the population, but account for nearly 40% of all hospitalizations. Renown Health, a leading hospital, aims to develop an interdisciplinary care plan and education process that enhances the overall pre-discharge and post-discharge experience for all older adult patients.

Member Duties : The goal of the Patient Education and Care Plan Coordinator VISTA position is to enhance health outcomes among hospitalized older adults in Washoe County, Nevada by aiding in the development of an interdisciplinary care plan process that enhances the overall patient pre-discharge and post-discharge experience for older adults. The VISTA member will work with the Chief Medical Officer and CEO of Renown South Meadows, a leading hospital in Washoe County, to conduct a community needs assessment, research evidence-based care transition plans, and identify opportunities to enhance patience competencies through education.
