Coordinated Entry Specialist
 
 
 
A core component of Family Support Center of South Sound’s mission and vision is Working Together, and that is realized through the organization’s many formal and informal partnerships. The Coordinated Entry system is a collaborative and coordinated effort to ensure our community’s most vulnerable homeless individuals and families get prioritized first for housing and shelter resources. The Coordinated Entry Specialist AmeriCorps position will be a critical component to ensuring homeless individuals and families have quick access to the system, and will be assessed and prioritized to receive the next available resource. The AmeriCorps position will increase the capacity of the system to be able to serve all individuals seeking Coordinated Entry support, and will work in a collaborative nature to best meet the needs of individuals and families. The full scope of this AmeriCorps project is to strengthen and support the established Coordinated Entry system in Thurston County in order to quickly and effectively address the unsheltered homeless population in our community. Currently, all three sites are operating on a shoe string budget and leveraging as many staff and community resources as possible in order to meet the incredible need and ensure the most vulnerable homeless households can access housing. AmeriCorps activities will includes: intake and assessment, referrals, safety planning, placement onto housing waiting lists and into various housing programs, and ensuring that in addition to the need for housing, the individual or family's basic needs are met (ie: food, clothing, hygiene supplies, transportation assistance, etc.). The Coordinated Entry system utilizes the evidenced based Housing First model; working to as quickly as possible identify housing and then, once housed adding layers of support to address the issues that led to the housing crisis. This position is contingent on funding.

Member Duties : The Coordinated Entry Specialist will provide intake, assessment, referral, and prioritization services for households seeking services at the three sites- Family Support Center, Community Youth Services, and SideWalk. The Coordinated Entry specialist will help to increase the agencies' capacity to serve homeless individuals and families with intake and assessment. The Specialist will primarily serve at Family Support Center, offering support 3 days per week. One day per week they will co-locate onsite at Community Youth Services serving youth and young adults, and 1 day per week at SideWalk serving single adults. The Coordinated Entry Specialist will be responsible for HMIS data entry, Coordinated Entry intake completion, utilization of population specific Vulnerability Prioritization Tools, and will remain up to date with the resources available to the various homeless populations.
