2018 Summer Coordinated Entry Liaison - DuPage County
 
 
 
Catholic Charities, Diocese of Joliet is a faith based organization providing service to people in need and calling others of good will to do the same. Catholic Charities has played a key role in the development and implementation of the DuPage County Continuum of Care’s (CoC) Coordinated Entry System. The goal of the project is to effectively outreach to persons experiencing homelessness to ensure they are provided with resources for which they are eligible through the County’s Coordinated Entry System. Coordinated Entry is housing-focused and designed to quickly connect families experiencing a housing crisis with housing resources as efficiently and effectively as possible. The CoC will partner with service providers that can assist the homeless with an assessment process and coordinate referrals for resources (such as prevention services, rapid re-housing, transitional or permanent supportive housing, emergency shelter and case management). The VISTA will be collaborating with other service providers to provide this assessment and other assistance as needed. This position is a part of the Housing Action VISTA Network. Member Duties : Expand the Coordinated Entry process for persons experiencing homelessness by providing assessments in a safe, neutral and private location. Provide increased engagement with persons experiencing homelessness by meeting them “where they are at” with no preconditions for involvement. Work effectively and constructively with persons from all ethnic, racial and social backgrounds; provide comprehensive assessment of participant’s ability/readiness to participate in housing stability service plans to meet the needs identified; develop and implement an individualized service plan that includes long and short term goals needed to assist participants with rapid economic recovery and maintenance of housing.
