2018 Coordinated Entry Case Manager
 
 
 
Ford Heights Community Services Organization (FHCSO) has been assisting vulnerable populations in times of crises, emphasizing emergency, supportive, and educational services to individuals and families until they stabilize, via its Coordinated Entry Program. The VISTA will work with the Coordinated Entry Case Manager Supervisor in connecting people in crises with resources, emphasizing on homeless prevention and diversion to homelessness. The end result of the project will be for FHCSO to increase the number of services provided to individuals and families to make homelessness a rare, brief, and one-time occurrence. The VISTA will ideally be an excellent team worker that possess ample computer/technology skills and sufficient written and oral skills in addition to a strong willingness to work with families that are homeless or at imminent risk of being homeless.This position is a part of the Housing Action VISTA Network. Member Duties : The objective of the VISTA is to assess, evaluate, and connect individuals and families to resources by completing the following activities: complete intake and assessment of clients that are referred to the agency, assist clients in collecting needed documents to qualify for housing opportunities, follow up with client’s as needed to close out referrals, research needed resources and refer clients for services, document case notes, and report to supervisor as needed.
