2018 Cornerstone Case Manager
 
 
 
Ford Heights Community Service Organization (FHCSO) has been providing emergency, supportive, and educational services to underprivileged families until they stabilize, via its Cornerstone Program. The VISTA Summer Associate will work with the Cornerstone Case Manager Supervisor to provide supportive services to low-income families who are formerly homeless or at risk of becoming homeless. The end result of the project will be for FHCSO to increase the supportive services provided to assist these families with returning to self-sufficiency. The ideal candidate will be excellent team workers that possess ample computer/technology skill and sufficient written and oral skills in addition to a strong willingness to perform community outreach with families that are homeless or at imminent risk of being homeless. This position is a part of the Housing Action VISTA Network. Housing Action Illinois administers 35 10-week VISTA Summer Associate positions located all throughout Illinois. Member Duties : The objective of the VISTA project is to assess clients to confirm if they qualify for the program and provide supportive services that are deemed necessary to move them to self- sufficiency by completing the following activities: complete intake and assessment of clients that qualify for the program, assist clients in collecting needed documents to complete their file, assist clients with completing a service plan with attainable goals and objectives, research needed resources and refer client for services, document case notes and report to supervisor as needed.
