THN Continuum of Care Coordinated Entry VISTA (Austin)
 
 
 
The Texas Homeless Network (THN) works to provide solutions to end homelessness in Texas communities through education, resources, and advocacy. Currently, THN has VISTA members placed throughout the state working on capacity building and program development to assist communities in ending homelessness. This VISTA project will build capacity to support the Texas Balance of State (BoS) ongoing implementation of Coordinated Entry (CE) system to better provide services for persons who experience homelessness. To achieve this goal, the VISTA member will primarily focus on supporting the Systems Change Coordinator by developing tracking and communication systems between the CEPEs (Coordinated Entry Planning Entities) and the BoS, strengthening partnerships and providing technical assistance. The outcome will allow community partners to more effectively provide housing and other services to persons experiencing homelessness.

Member Duties : 1.) Familiarize with HUD CE requirements 2.) Familiarize with existing BoS CE program written standards, CEPE (Coordinated Entry Planning Entity) Policies and Procedures, identifying strengths and gaps in system and BoS support. 3.) Familiarize with current data collection, reporting, sharing, training and databases/tools used. 4.) Organize and participate in application of new and enhanced processes. 5.) Work with initiative partners and CEPEs. 6.) Assist in creating enhancements for data collection and performance. 7.) Evaluation of CEPE coordinated entry progress and overall system 8.) Identify and document CE improvements and continued needs related to Diversion, eligibility, referrals, improving data management and strengthening community partnerships 9.) Present findings to THN 10.) Create a transition plan for sustainability.
