NYC VISTA - Network of Care VISTA
 
 
 
The NYC DHS Office of the Medical Director works to improve the medical and behavioral healthcare of all homeless individuals in New York City to increase their chances of succeeding in permanent housing and independent living. We accomplish this goal through a variety of evidence-based programs that are rigorously tested, evaluated, and adapted to meet the particular needs of the NYC homeless community. OMD has the unique opportunity to work and partner with a variety of collaborating agencies, offices, and departments to achieve our mutual goals of helping homeless individuals. The 2018-2019 VISTA will be instrumental in moving the Network Project forward through to fruition. This project was conceived as a four-part project to get healthcare access for all DHS clients via shelter based primary care, urgent care, neighborhood clinics that can continue to work with the client once they are housed, and city hospitals. Expected outcomes are improved health, quality of life, and well-being for clients, increases in housing placement, and decreases in the rate of emergency departments and inpatient hospital visits. This exciting new initiative will involve canvassing shelters, geo-mapping, piloting programs, focus groups, rigorous evaluation, and quality improvement, all with the goal to improve health outcomes and decrease the mental health burden for homeless populations in New York City. This position is ideal for the VISTA candidate who wants to work with vulnerable populations, is interested in seeing all the phases of program development including project exploration, implementation, and evaluation, and would like to gain valuable quantitative skills and experience in data collection and analysis.

Member Duties : You will support the Office of the Medical Director to map existing behavioral health, medical, urgent care, and hospital facilities in New York City and their relationship to previously mapped shelters. These mapped locations will be used to create clusters and partnerships between shelters and healthcare facilities. You with be instrumental in developing, implementing, and evaluating a pilot for the Network Project, including but not limited to developing the network workflow for referrals from shelters to providers, conducting assessments and project planning, literature reviews, interviews, and evaluation of the success of the pilot.
