MediCorps - Summer - Health Coach/Case Management Organizer
 
 
 
The Viola Startzman Clinic is a vibrant safety net clinic serving the most vulnerable patients in Wayne and Holmes Counties. Our team of passionate staff and volunteers thinks outside of the box to meet the needs of our neighbors. The VISTA member who accepts this position will join our current team along with 3-4 other VISTA members in helping the clinic to better meet the needs of our low income, complicated patients. Our patients come needing far more than a diagnosis and a prescription. They need case management to address issues such as lack of food, housing, transportation or emotional support. They also need help following through on medication compliance, diet and lifestyle changes and overall treatment plans. This VISTA position will help address the needs of patients from the time they check in to the time we celebrate a successful end of treatment.

Member Duties : This VISTA member will be working with community partners and volunteers to research and develop two programs: a health coach program that utilizes volunteers and health students to help provide follow up and coordination of chronic condition care; and a Case Management Intake process to help develop a consistent screening tool to identify all needs of a patient including social services, drug usage, domestic violence, depression and anxiety, suicidal tendencies and other patient needs. For both programs, the VISTA member will be connected with successful models in other organizations as well as professional resources to help develop the programs. The VISTA member will not only develop the program, but train a team of volunteers to administer the program at the end of the summer.
