Healthy Futures – Health Promotion Council
The Chronic Disease Management and Prevention Outreach, Recruitment and Training Project will address chronic disease and poverty in the most vulnerable sections of Philadelphia, which includes the more densely urban, residential zip codes. The rate of diabetes for the African-American population in the targeted geographic area is 18.8%, the highest in Philadelphia. In addition to the challenges facing these patients with their diabetes, they are also affected by other chronic diseases that complicate their diabetes and reduce their overall health. These are overweight/obesity (56% patients), hypertension (17.8% patients), and tobacco use (17.8%). Patients also struggle with a variety of other health related needs such as poor nutrition and mental health. In addition to their health related challenges, patients also struggle over concerns such as sustainable employment. The Chronic Disease Management and Prevention Outreach, Recruitment, and Training Project will partner with PHMC’s Health Network (PHMC HN) along with other partners to address the problem of uncontrolled diabetes and other chronic diseases among Philadelphia’s high risk populations.

Member Duties : 1. Using best practices and evidence from previous grantees and research methods to establish an outreach and recruitment protocol for the identified zip codes to attract participants for the chronic disease self-management and prevention programs offered. 2. Establish a list of current and potential recruitment partners 3. Establish/refine a protocol and process for conducting outreach and recruitment. 4. Train potential interns in outreach and recruitment procedures, as appropriate. 5. Identify community outreach events and establish a contact list for future use coordinating multiple outreach opportunities for HPC programs. 6. Recruit and enroll at least 300 participants in chronic disease self-management and prevention programs in Philadelphia. 7. Establish a plan for post-program participation. 8. Recruit lay leaders, using an established lay leader recruitment process, to be trained by HPC Master Trainers.
