Diabetes Program Development VISTA
 
 
 
The Program Development VISTA (PDV) will work with the San Angelo Diabetes Coalition (SADC) to develop new, collaborative, community-wide services for management and prevention of diabetes. New services developed and implemented by the PDV and the coalition team will improve collaboration and effectiveness of diabetes self-management education services provided through local community clinics and two regional hospitals. The new services developed by the PDV will increase access and participation in effective, evidence-based, diabetes self management education services for low-income and uninsured individuals who have diabetes or high risk of diabetes onset. The partnership with SADC is through the Angelo State University, Center for Community Wellness, Engagement & Development.
