Bridges Health Winona is committed to serving the community while educating students. Bridges addresses gaps in healthcare for low-income communities while responsibly preparing future health professionals to serve underserved populations. Bridges works with individuals and families without access to preventive health services and provides health screenings and exams, health teaching, promotion, and coaching; linkage to community resources; and active living groups. This is done through an interprofessional practice team approach that expands the expertise and quality of our services. Specifically, social work, nursing, exercise science, counselor education, and medicine are involved. Evidence-based practices of motivational interviewing, patient- and relationship-centered care, and antibiotic stewardship are used. Students lead the care provided, while being supervised by faculty and clinicians. Individualized care is possible due to the amount of time students have with the patients and the access to invaluable academic resources. The VISTA will focus on capacity building projects, including 1) expanding reach of services to reach hard-to-reach populations, 2) enhance design of leadership model, and 3) advance diversity and efficiency in management of students and volunteers. ---This project is part of the College Health Corps (CHC) VISTA Program, operated by Minnesota Campus Compact. The CHC increases low-income individuals’ access to health care, health education, food, and basic needs through capacity building projects, including partnerships between higher education and community based organizations. Professional development focuses on health equity, the social determinants of health, and effective partnership development. --- ***Please also send cover letter and resume to supervisor. Member Duties : The AmeriCorps member will work closely with the program coordinator and team members to provide capacity building efforts focused on sustainability. Three foci describe this work: Project 1: The member will explore, design, solicit, train, and pilot volunteer and community partnerships to expand the reach of services to more people; this will involve community organizing and needs assessment. Project 2: The member will research, design, propose, and implement a team leadership system, developing supporting resources such as templates for grant writing and research on funding options for sustained infrastructure; this will involve facilitating discussions and planning with the leadership team. Project 3: The member will design and implement mechanisms and systems to diversify student involvement and provide efficiency in management of the program to support sustainability; this will involve developing new partnerships and implementing new processes.
