This VISTA project aims to meet health needs by increasing access to healthcare for economically disadvantaged individuals. As these individuals most often have Medicaid or Medicare, it's more difficult to obtain coverage for medically necessary medications, diagnostics procedures, and services. This lack of access to care contributes to health disparities and puts low-income individuals at higher risk of complications from chronic disease progression. This project will build upon a pilot program in which we created a position called Patient Services Advocate that assists with completing tasks related to obtaining approval for the treatment of Hepatitis C. With this pilot we have seen an increase in the number of patients who have been treated for Hepatitis C, and they have been able to do the treatment with their primary care provider instead of having to be referred to specialist. As payment models shift toward pay-for-quality over the next few years, we anticipate that having this type of position at our health centers will become sustainable through quality payments that reward providers for improving patient health outcomes and reducing overall healthcare expenditures. The VISTA members will be able to support this through continued development of systems that streamline the process from when a provider orders a medication/procedure/service, through getting it approved, and then scheduling/following-up as necessary to ensure the patient receives the service. Additionally, they will assist with developing a documentation process that ensures that patients are informed throughout the process and feel empowered in their healthcare. We will have two VISTA members who will work across our six FQHC's (Federally Qualified Health Center), with the overall goal to create a sustainable model that is cost-effective.

Member Duties : 1. Establish relationships with medical offices and insurance companies. 2. Establishing a system for requesting prior authorizations for non-formulary medications and diagnostic procedures. 3. Formalize the process to complete insurance and medical forms needed for social services and other services that support patient wellbeing. 4. Facilitate the submittal of record requests to other medical facilities and specialist to obtain necessary results/reports. 5. Creating a training system to appeal denials to insurance. 6. Build and maintain strong effective communication and integration of services. 7. Complete Certified Application Counselor for the Health Insurance Marketplace training and conduct outreach presentations 8. Coordinate programming of 20+ special events for patients and staff with support of the Social Services Manager and Health Care Administrator. Coordinate programing for special events with the support of staff.
