FIT VISTA - Community Health Worker Program Specialist
 
 
 
Dartmouth-Hitchcock Community Health works with health care professionals and community partners to improve the health of people living in the communities we serve to ensure all citizens, including low income have access to care. Together with public health organizations, town leaders, schools, police, and community organizations, we complete Community Health Needs Assessments every three years, then work together with these partners to change the behaviors and community conditions that have the greatest impact on health, as detailed in our Community Health Improvement Plan. Along with responding to specific local community needs in communities where we operate Primary Care clinics, Dartmouth-Hitchcock's Population Health Council reviews community health needs assessments across New Hampshire and Vermont, and has identified three priorities for community health initiatives across our health care system. These priorities for our work include: Improving outcomes related to substance use and mental health needs; Improving health and wellbeing of older adults; and Increasing access to well-coordinated care.

Member Duties : Coordinates multi-stakeholder community partnerships to plan and implement population health initiatives of the D-H Department of Community Health Improvement; Drives the development of one or more community partnerships comprised of an array of health care, municipal, education, community service, business, and law enforcement members; Ensures project related assessment of community needs and assets as assigned; Coordinates with guidance from senior staff, assessment, capacity development, action planning, implementation, and evaluation of projects by community stakeholders and organizations; Supports the implementation of indicators to track progress of regional strategic wellness plans toward goals; Provides support and technical assistance to community partners to achieve the goals and objectives for which their organizations are responsible; Assists with and independently conducts evaluation to track progress and improve the quality of future work.
