The Full Life Care AmeriCorps Team addresses a constellation of challenges associated with social isolation. Team Members are placed in positions throughout a network of services for older adults and adults with disabilities to facilitate one-on-one and group activities and outreach efforts that engage individuals in effective, evidence-based programs that promote health, wellness, and independence through increased physical activity and social connection. One member will serve at Full Life Care’s Adult Day Health Center in Kent, WA. Full Life Care is dedicated to enhancing the quality of life for frail elders and people with chronic or terminal illnesses and disabilities. At Adult Day Health, participants receive group exercise classes, cognitive activities, and support to help them remain as healthy and independent as possible. Member Duties : The Health Promotion and Volunteer Engagement Specialists will serve at our Adult Day Health Center in Kent. Duties include: Develop and facilitate therapeutic group activities; Lead exercise and cognitive function groups; Implement the Chronic Disease Self Management Program (CDSMP) Workshops. Training provided; Implement a Healthy Living activity group and /or coordinate monthly health education presentations; Act as liaison between program staff and Full Life’s Volunteer Coordinator. Communicate with team and supervisor to identify needs and collaborate with Volunteer Coordinator to identify and implement strategies to recruit new volunteers; Greet and train incoming volunteers; engage and support volunteers in client group activities; track and report volunteer hours Additional duties: Administer and collect data to meet Healthy Futures performance measure; Attend team meetings and service projects.
