Sunrise Community Engagement VISTA
 
 
 
Sunrise is searching for a VISTA candidate interested in improving the health and wellness of uninsured and under insured individuals. The assignment will focus on building programs that enhance the current chronic disease management and prevention program, educate patients about Sunrise services, and research resources that will benefit our patients. Sunrise Community Health began in 1973 as a migrant health clinic. Today, our family medicine, pediatric, prenatal, dental, and school-based health centers are health care homes for more than 34,000 people in northeastern Colorado. We are a private, non-profit 501c3 supported in part by community health center funding through the Bureau of Primary Health Care. Sunrise offers comprehensive, quality, and affordable medical, dental, and behavioral health services including on-site laboratory, radiology, pharmacy, and patient education services. All services are offered to people in need regardless of their ability to pay. Sunrise is a recognized leader in integrated care, advanced Health Information Technology (HIT), safety net collaborations, and professional health education. In addition, Sunrise co-founded the North Colorado Health Alliance, a community collaboration focusing on the low-income, under and uninsured people within our service area. Partners include community health, public health, hospital, behavioral health, specialists, local foundations, education, county commissioners, managed care organization, and county social services. Collaborations focus on integrated service expansion; shared HIT infrastructure; workforce development; community health improvement; regional accountable care activities; and system accountability and efficiency. Our mission is “Supporting the health of individuals, families and our community through affordable, quality care and education”. We strongly believe that healthy people equal healthy communities.

Member Duties : The goal of the project is to break the cycle of poverty by supporting Sunrise Community Health’s community outreach projects, health educational programs, and quality improvement initiatives to help uninsured and underinsured individuals achieve better health outcomes. The VISTA will develop wellness educational materials and initiatives, create a volunteer program for Sunrise programs, obtain donations for the Migrant and Homeless Outreach Program, and create community awareness of Sunrise’s services. Sunrise will provide a professional environment where a VISTA will be able to hone their skills in project coordination, communication, creativity, developing community relationships, and teamwork. By having a VISTA, Sunrise will be able to outreach and provide education to more patients. It is the hope of Sunrise that these endeavors will ultimately improve the wellness and health outcomes of the population that Sunrise serves.
