The mission of the Department of Family & Community Medicine (DFCM) at the University of Arizona (UA) is to advance the health of individuals and families from all backgrounds and cultures through innovation and leadership in education, research, community engagement, and the delivery of exemplary primary care. Founded in 1967, the DFCM is one of the original departments within the UA College of Medicine. We have four, interrelated mission areas: education, patient care, research and outreach. Our department is known for its outstanding pre-doctoral and post-doctoral education preparing the next generation of family physicians, increasing diversity in the profession and helping to expand the primary care workforce in underserved communities. The Practice Transformation Project builds on work accomplished by previous VISTA members who worked to develop and implement Quality Improvement strategies that inform team-based care in DFCM clinics. This project focuses on developing, improving and implementing PCMH key functions and core competencies in order to deliver quality care focused on keeping patients healthy and based on patients’ needs and preferences. The VISTA member in the role of Practice Transformation Coordinator will be working with the medical directors, faculty physicians, residents and clinic staff to improve the following functions: Team-based care and practice organization, knowing and managing your patients, and performance measurement and quality improvement. **Arizona Serve accepts applications on a rolling basis. Member Duties : Work with Medical Directors to establish the Care Coordination Team that will develop strategic plans related to quality improvement, work with the Care Coordination Team to develop strategies for population management of our patients to improve quality of care and health outcomes, determine opportunities for funding care coordination team positions, and working with community partners, insurers, local and federal governmental agencies, and philanthropic organizations to seek funding to staff on-going care coordination team, manage and validate quality data through work with the electronic health record and other staff members to identify access to patient-oriented data, collaborate with clinic staff, lead and staff the current multidisciplinary Quality Improvement Team by facilitating meetings and committee work, and review data periodically to assess quality improvement strategies and workflows as well as outcomes of clinic changes.
