2017 Lake County Homeless Continuum of Care
 
 
 
The Lake County Continuum of Care services nearly 1500 people in Lake County who experience homelessness each year; Lake County staffs the efforts of the Lake County Coalition for the Homeless’ (LCCH) efforts to end homelessness through collaboration, capacity-building and system change. LCCH is launching a Social Security Outreach, Access and Recovery (SOAR) Initiative to increase the income of disabled homeless people. LCCH completes an annual point-in-time count of unsheltered homeless to track progress on ending homelessness. The Homeless Continuum of Care Program VISTA will has two objectives: plan and coordinate the 2018 Unsheltered Homeless Point-in-Time (PIT) Count, and increase the income of approximately 10 disabled homeless people through SOAR.

Member Duties : Homeless Continuum of Care VISTA will: (1) Increase the number of SOAR applications tracked in the Online Application Tracking (OAT) database. (2) Plan and implement the 2018 Unsheltered Homeless Point-in-Time (PIT) Count.
